Same-day & Walk-in Appointments

Sports Physicals are an essential (KSHSAA required) assessment for athletes. They are not only used to determine if an athlete is healthy enough to play sports, but is also used to evaluate and prevent the risk of future injuries. With every Sports Physical we go above-and-beyond what is necessary to assess and document by including a complimentary Functional Movement Screen.

The Functional Movement Screen (FMS) is a screening tool used to evaluate seven fundamental movement patterns in individuals with no current pain, complaint or musculoskeletal injury. The FMS is not intended to diagnose orthopedic problems but rather to demonstrate opportunities for improved movement in individuals. Improved movement may decrease the risk of future injury.

FMS Screen: Deep Squat; Hurdle Step; In-line Lunge; Shoulder Mobility; Active Straight Leg Raise; Trunk Stability Push-Up; Rotary Stability

If the athlete has a history of any type of heart problem, heart condition, or an episode or event related to their heart, we require documentation from the athlete's primary care or treating physician stating the condition has been evaluated and does not prevent participation in sports.

Please dowload, print, fill out and bring in the following Pre-Participation Physical Evaluation form.
