Costovertebral (Rib) Joint Dysfunction

Many patients come to our office saying they have a "rib out of place." In most cases, the rib hasn't actually dislocated. Instead, the pain is often caused by costovertebral joint dysfunction—a condition where the joints connecting the ribs to the spine become irritated, restricted, or inflamed.
These joints play an important role in breathing, posture, and upper body movement. When they become restricted, even simple activities like taking a deep breath, twisting, coughing, or reaching overhead can become painful.
At Midwest Muscle & Joint Clinic, we provide evidence-based, conservative treatment to restore normal rib movement, reduce pain, and help you return to your daily activities as quickly as possible.
Every treatment plan is tailored to your symptoms and examination findings. Depending on your condition, treatment may include:
Chiropractic spinal and rib adjustments to restore normal movement of the ribs and thoracic spine
Manual therapy to reduce muscle tension and improve soft tissue mobility
Shockwave Therapy (ESWT) to address chronic muscle dysfunction and promote tissue healing when appropriate
Dry needling to reduce muscle spasms and painful trigger points involving the intercostals, rhomboids, paraspinals, or surrounding musculature
Instrument Assisted Soft Tissue Mobilization (IASTM) to improve tissue mobility and reduce fascial restrictions
Corrective exercises to improve thoracic mobility, posture, and upper back strength
Personalized home mobility program to reduce stiffness and prevent recurrence
Our goal is to restore normal movement of the rib joints while improving breathing, posture, and overall function.
Common symptoms include:
Sharp pain between the shoulder blades
Pain along the ribs
Pain with deep breathing
Pain when coughing or sneezing
Pain with twisting or reaching
Localized tenderness near the spine
Muscle spasms in the upper back
Pain that feels like a "rib is out"
Many patients notice the pain is worse after sleeping awkwardly, lifting, golfing, throwing, or prolonged sitting.
Costovertebral joint dysfunction may develop from:
Poor posture
Heavy lifting
Twisting injuries
Sports involving rotation (golf, baseball, tennis)
Repetitive overhead activities
Persistent coughing
Falls or trauma
Muscle tightness
Thoracic spine stiffness
Although patients often describe a "rib out of place," true rib dislocations are uncommon outside of significant trauma. More commonly, the rib joint simply becomes restricted or irritated, creating pain and muscle guarding.
Your initial evaluation at Midwest Muscle & Joint Clinic is designed to determine whether the rib joints are contributing to your symptoms and rule out other possible causes of thoracic pain.
Your visit may include:
Detailed health history and symptom review
Thoracic spine and rib examination
Orthopedic evaluation
Postural assessment
Functional movement evaluation
Personalized treatment recommendations
If your symptoms suggest a rib fracture, lung condition, cardiac condition, or another medical concern, we will recommend the appropriate imaging or referral.
Our chiropractors evaluate the ribs, thoracic spine, shoulders, and surrounding muscles to identify the true source of your pain. By restoring normal rib and spinal mechanics while addressing soft tissue restrictions, we help patients recover quickly and reduce the likelihood of recurring episodes.
Your treatment may include chiropractic adjustments, manual therapy, Shockwave Therapy (ESWT), dry needling, IASTM, and corrective exercises.
Our treatment plans are designed to:
Reduce rib and upper back pain
Improve breathing comfort
Restore thoracic and rib mobility
Improve posture
Reduce muscle spasms
Help patients avoid unnecessary medications whenever possible
